Fed: Houston appoints senior army office to head inquiry

A senior army officer is to head an inquiry into how the body of Private JAKE KOVCO
was left in Kuwait and another body flown to Melbourne instead.

Defence head Air Chief Marshal ANGUS HOUSTON says a quick assessment of what went wrong
was conducted yesterday .. and that will provide the basis for the inquiry's terms of
reference .. starting next week.

PRIVATE KOVCO .. a member of the Australian Army security detachment in Baghdad ..

died last week when his pistol accidentally fired.

His body was flown on a RAAF transport plane from Baghdad to Kuwait .. from where it
was to have been flown to Australia.

Instead .. his body was left behind in Kuwait and a casket containing another soldier's
body was flown to Melbourne.
